Quest Policies

Consent to treatment

By attending an appointment at our clinic, you acknowledge that you willingly give consent to our Physiotherapists to perform any and all necessary examinations and treatment procedures in accordance with their professional training, qualifications and understanding of your injury.

You understand that, during your treatment, different Physiotherapists at Quest Allied Health Pty Ltd may be involved in your care.

You understand that you have the opportunity to discuss the nature and purpose of your physiotherapy care, with your Physiotherapist, before any treatment is performed, and that you may opt out of treatment at any time.


You acknowledge and accept the following:


Treatment
  • Physiotherapy involves physical treatment, which may include “hands on” treatment, among other treatment options.

  • You have the right to choose the Physiotherapist that you wish to see, refuse treatments offered to you, or seek a second opinion at any time.

  • There are possible risks associated with physiotherapy treatment modalities, including but not limited to; stiffness and soreness, soft tissue injury, neurological complications, cerebrovascular injuries, skin irritations, burns and other minor complications.

  • The chance of risk is lowered by a thorough assessment of your condition, and we take every precaution to minimise risk to you and to the therapist.

  • In many cases, other treatment options may be advisable. If the risk of treating your condition with physiotherapy modalities is considered to be too high, your therapist may refer you on to other forms of care. This on-referral will also occur if they believe your condition is outside of their scope of practice.

  • There may also be an inherent risk of remaining untreated. Depending on the requirements for your treatment and condition, remaining untreated can result in adverse effects to your health and continued deterioration of your symptoms.


Consent to Obtain and Release Information
  • To speed up processing and management of your injury or condition, you accept it may be necessary to communicate the details of your medical condition with treating practitioners, rehabilitation consultants, case managers, and solicitors that are involved in your case.

  • By signing these Terms and Conditions you give permission to Quest Allied Health Pty Ltd to exchange such information about your injury, condition or claim, as well as significant past medical history, with professionals deemed necessary by Quest Allied Health Pty Ltd. 

Your privacy is critical to us.

Likewise, we have a comprehensive Privacy Policy (clause 4), which includes details of how we gather, utilise, impart and reveal individual data.


Exclusion of Liability

The director of the practice ‘Quest Allied Health Pty Ltd’ and any employees of this practice shall not be held liable for, nor accept any responsibility for any injury, loss or damage sustained by any person or persons resulting from treatments or procedures delivered in this practice or by our physiotherapists or in any way whatsoever which does not arise from any negligence of the practice director or the practice employees.


Privacy and Contact
  • Quest Allied Health Pty Ltd will send you SMS or email confirmation for appointments

  • You may, from time to time, receive marketing or surveys from Quest Allied Health Pty Ltd. These are used to improve our services, and to offer our customers promotions and discounts.

  • You have the right to opt out of any SMS or email contact at any time.

  • Your details will never be shared with a third party except in cases where details are used to support you in your claim or injury, as outlined in clause 2.


Payments & Billings
  • You are required to pay for the services provided, and any items provided at the time of your consultation.

  • Payment can be made over the phone, online (specific services only) , in person when your Physiotherapist comes to you, or at the clinic where you attend your session. Other payments can be discussed with the admin staff, and may be available at their discretion.

  • There is no refund available on the purchase of any personal or healthcare items for change of mind. If the item has a major fault we will refund or exchange the item for you. If the problem is not major, we will repair it for you within a reasonable timeframe.

  • Our strict billing methods enable us to continue providing high quality treatment for all of our patients, and are not designed to exclude or discriminate against any persons or groups of people.


Late arrivals, Missed appts, Cancellations
  • If you arrive late to your session, but within your appointment time frame, you may still receive treatment for the remaining session time.

  • If you miss your appointment or arrive outside of your appointment time frame you will be charged a cancellation fee of $50.

  • If you are able to organise a rescheduled appointment within the same business day, no cancellation fee will be charged. This is subject to availability of our clinicians.

  • You may cancel or reschedule an appointment so long as you provide us with a minimum of:

  1.  24 hours notice prior to your appointment time (Private, CHSP, HCP funded).

  2. 48 hours notice prior to your appointment time (NDIS, GP Referrals).

  • Cancellations within the 24/48-hour period will be charged as per clause This cancellation fee may be waived for genuine extenuating circumstances and at the discretion of practice staff.
